The public request form lets tenants, occupants, and staff report maintenance issues without a FacilityLane account. You share it as a URL or a printed QR code; submissions arrive in Requests as Pending for your team to review.

Set up the form

Open request settings

Go to Requests and open the Public Form settings.

Configure the form

Choose the categories and locations submitters can pick from, so incoming requests are routed and filtered cleanly. Keep the lists short and written in plain language your occupants recognize (for example, “Restroom” rather than internal asset codes).

Save and publish

Save your settings. The form is live at its public URL immediately.

Open the share options

In the Public Form settings, click Share. You get the public URL and a QR code for the form.

Distribute the URL

Post the URL on your tenant portal, intranet, or email signature so people can submit from any device.

Print and post the QR code

Download and print the QR code, then post it where issues happen — lobbies, break rooms, restrooms, near equipment. Scanning opens the form in the phone’s browser; no app install is needed.
Print the QR code with a short instruction line such as “See a problem? Scan to report it.” Response rates are much higher when the poster explains what the code does.

What submitters see

When someone opens the link or scans the code, they see a simple mobile-friendly form. They fill it in and submit — no login, no account creation.

Submitted fields

After submitting, the person sees a confirmation screen. The request appears in your Requests queue as Pending.

Submitter-selected priority is a signal, not a commitment — your team can reprioritize during review before converting to a work order.
The form URL is public by design. Anyone with the link can submit, so review requests before approval — see Managing Requests for triaging and rejecting invalid submissions.
